Add a Formstack connection and a trigger for new form submissions. Select the form that should create tasks.

Add an Asana connection with permission to create tasks in the destination project.

Map the task

In JavaScript, set the task name from a key answer or the submitter name. Put the remaining answers in the notes. Create the Asana task in the project you choose. Submit a test entry and confirm the task.
